Referral
An application can be requested by phone, fax or e mail. All applications are processed by Melissa Stiegler. Applications should be filled out as completely as possible. Each application is reviewed by the director and staff. Decisions are made within 5-7 working days. We would be glad to provide you with additional information on the Chapter and to meet with parties personally to discuss their situation. All information is confidential. There is no charge for the initial intake process.
